A well restored hairline serves the main objective in hair transplant - to look younger and better. It bears the signature of the surgeon. Any fault can be easily spotted. By setting the front border of transplanted area, it determines the number of grafts.
Facial Attractiveness
We judge by pattern recognition. The 3 basic criteria for facial attractiveness are symmetry, balance, and well proportion.
Da Vinci's Golden Rule of Thirds
In Western countries the proportion of beauty is Da Vinci's Golden Rule of Third, stating that an ideal face should have equal vertical distance from :
- Lowest point of hairline to Glabella
- Glabella to base of nose
- Base of nose to tip of chin
In hairline design the anterior-most point is therefore set accordingly.

Exceptions To The Rule of Third !
Although this rule has been mentioned and followed by most hair surgeon, we find it impossible to apply to all individuals.

Case 1: A Smaller Jaw
This happens very often, especially amongst the Chinese. You can see now that the proportion of the hairline has now changed to a lower level.
Case 2: A Longer Chin
This happens more in Caucasican with a musculine look, like action hero. Now that the proportion of the hairline has also changed.
 Our Soltion to These Variations
After analysising the facial proportion of hundreds of celebrities, we have found a new formula to locate the hairline.
The result was presented in the 2009 ISHRS Annual Scientific Meeting in Amsterdam, Netherland.
The presentation was reported as one of the "Finest" in the meeting.
We are using these formula in our daily work, and found very pleasing results.

4 Other Rules in Hairline Design for Men
- The apex is directly above the outer corner of the eye
- The apex is not rounded
- Hairline when looked at from front is an inverted U
- Hairline when looked at from side is horizontal
Other Considerations in Hairline Design

Gender

In men the hairline is an U shape with shape-corner apex
- In women it is an inverted U shape with rounded apex
Facial shapes
The hairline must match the different shape of the face
- Triangular: avoid very straight hairline
- Square: avoid bell-shape hairline
- Oval or Round: avoided a U-shape hairline
Slope of forehead
- Flat: lower the forehead may make no visual impact
- Cliff like: lower the hairline and the hair would cover the face
- Slope: the point where the slope change from vertical to horizontal set the anterior-most point
Good old memory and expectation
Some patients like to bring along an old photograph, while others bring along pictures of favorite movie stars and celebrities. All must be taken into reference.
Budget
When the hairline is lowered by 1cm, the transplanted area increases by 8 sq.cm and number of graft by 400-500 FUs.
Number of Donor Hair Available in that session
Always go for a more conservative hairline if limited donor supply.
